Bought it from the mint directly on an auto order. three sets and this is the only one that came this way.
it appears that all the quarters are undamaged and the extra is an additional Salt River Bay quarter.

Just hold onto it, it is interesting mishap by Mint, but it won't really fetch any noticeable premium.

Appears to be a proof set - concur with above advice that's its unusual but not a valuable find.
You have an extra proof mint quarter or two - value is whatever they are worth.

Shhh. Don't tell the mint that you got an extra quarter in the set. They might charge you extra for that!

I would open the lens to get the extra coin out. It's very easy to do.

Leave it that way. It was a bonus coin sent to you by the Mint in appreciation for your purchase.

Exactly. Since 1999 the proof set cases haven't been sealed so opening and closing them with no trace has been possible. Which is why back in 1999 there was a flurry of "error" sets with a duplicate quarter and one missing. Buy five sets, open them, play musical quarters, and sell the "error" sets on ebay for $150 or more apiece. Then in 2000 there were "special VIP" ten coin 1999 proof sets that included the 1999 P SBA. Take a 2000 set, remove the coins and replace with the 1999's and put back into a 1999 box. Those reached close to $1000 before everyone learned about the scam.

The extra quarters helps just a bit with the price increase for these sets.

I do remember a proof set of 5 quarters on ebay, with 5 Effigy quarters in it. The cases are easy to open. Needless to say I did not buy it. But I'm sure someone did

These things do happen. I never saw one doubled in a hard plastic holder though. Here's my bonus coin
