This could happen if someone tossed a full bag of Morgans onto a pile of them - that's 60lbs of silver falling onto another similar bag and such an impact could drive one coin into the next one with a bunch of force. Anything that happens to the coin after the completion of the strike is PMD. Doesn't matter if it happened inside the mint building or after it left the building.
