LMC 1988 has this error on the reverse ..How could it happen..?

I look over this 1988 LMC and saw it through my USB Scope..That is amazing if this is nature caused

I am thinking they are talking about the first you in pluribus... there is a break in it.
Looks like PMD to me... but I am not sure!

Probably some grease in the die. That would prevent the full strike. Or it maybe just a contact mark?
