My initial reaction is that someone pressed (vise job) a Lincoln Cent reverse onto the obverse of a nickel, and also filed-off (sanded off?) the reverse of the nickel. Question: Is any of the Lincoln's reverse lettering showing on the nickel? If yes and the cent's lettering is reversed, then that's a clear clue that this was a vise job (or mallet job).

Others will weigh-in, I'm sure.

Looks like a vise job. If that is the ops nail you might have fungus better get that checked.
