We know there was a mistake that caused over a million notes to be printed with the same serial number in FW and DC. So duplicated serial numbers do happen but they are supposed to be unique. If I post a random note the chance of it being unique is almost 100%, so using 6 sigma all serial numbers are unique minus a few outlier or error.
