Looks like machine (strike) doubling to me, flat, shelf-like surfaces on the letters/numbers.

I don't find any references at all to a DDO 1909 VDB. Neat MD though!

A doubled die error occurs at the hub stage. It shows itself as one fully-struck image adjacent to another fully-struck image. Your underlying images lack the depth of fully-struck images, and are strike-doubling. Pretty dramatic, though, I'll give it that.
