I was sorting a bag of half dollars that I have by date and came across this? What would be the cause of all of this mess? The Reverse seems fine.

There's strong Machine Doubling and a weak strike. Could be caused by inadequate striking pressure, or it could be an improperly annealed planchet.

IMO, it's not post-mint trauma. The weakness of the strike simply meant that not enough metal flowed upward to fill that part of the die.

Got it! Thank you so much, is this anything to keep or just throw it into the rolls with the others?

This coin is a ' Greaser' - Struck Through Grease obstructed dies. Although it is not highly valuable or even all that uncommon, it was DEFINITELY caused by crud obstructing the obverse die in the minting press. This is a VERY clear case of Grease Filled Dies, and the photos very clearly show it.

Pretty strange on the dent in the head, but a cool find. Greasers are nice to collect.
