Looks like it was in a "Good Luck" holder and someone pried it apart. Thus the gouge at 9:00.

I'm sorry, what is it that you think might be an error? All I can see is the damage near "LIBERTY", damage to the rim on the reverse, and maybe some spots on the reverse that were Struck Through Grease...unless I'm missing some detail in the photos.

the rim on both sides is "threaded" looking.

Ok, I see it now. I agree with coop. The worn spot on the reverse coincides with the worn area on Lincoln's head on the obverse. Could be the point where the applied pressure to get it into the holder.
