I'm not 100% certain, because it's kinda unusual, but I believe it's PMD.

Looks like it could have been struck through a feed finger. The edges around the area are pretty sharp and there doesn't seem to be any metal movement.

It looks like a planchet defect to me, most likely pre-strike damage. The weakness of the designer's initials supports the idea that the deficit was present before the strike.
