I also think you're on the right track with a mishap during rimming. It's amazing how much Squire Wilson's dollar looks like mine. And even Nancy's shows a lot of similarity.

I might try and research some info on how the rimming is done. I think maybe the mint calls it the milling process? Unless any of you have some knowledge of it already?

Andrew Crellin suggested I contact Ian McConnelly who writes articles for CAB mag. to see if he could help, but I don't own any copies of it (as yet), presuming it gives some contact info. Didn't find any helpful info when I googled his name either.
