1979 Quarter has a completely filled in D, Not sure what's going on with his neck(looks like metal scraped up and stuck), and does anyone else see that faint E? Can anyone explain these errors

The post of the "D" mint mark broke off. Very common. The other two marks you speak of IMO are PMD. Something hard and pointy scraped the coin and made the gouge. The "E" just happens to be damage in the shape of an "E"...

On a Die that strikes the planchet everything is reversed...so instead of the "D" being raised..it is incuse or sunk in....the post that would normally make the middle of the "D" has broken off due to striking hundreds of thousands of quarters under extremely high pressure. This happens quite alot.
