1989-P Nickel. "Jefferson's Bad Hair Day" What Caused This?

I don't know what caused this extra "blob" of metal above Jefferson's head. The die appears to be in a pretty late stage. Could this be what caused it? Is the problem with the word "Trust" due to die attrition? Any help is greatly appreciated.

I see a coin from a VLDS die. Could be the design of the top of the head flowing toward the rim. Be the first time I've seen that though.
