The designers initials are always a part of the design as hubbed into all dies. They cannot be "forgotten", they cannot be "missed". What causes them to become missing is overpolishing of the die in the area of the initials. Because they are very small and are close to the edge of the design, for them to go missing after a die wears or is polished is completely understandable and at least somewhat surprising that it doesn't happen far more frequently. As in the normal minting process? Then my assumption would be that it wouldn't carry a premium, but that seems just the opposite in this case. Is it because it's just not something that is commonly looked for? Knowing the government and the fact that they will try to squeeze everything they can out of something, you would think these over-polished dies would be used until they break, so there would be a lot of these floating around. It might only happen once in twenty years, but it does happen. As to premium value - because of the nature of their making, I would never pay a premium value for missing details on a coin due to overpolishing, but there are a lot of other people who would.
