Slabaugh's "Specialized Catalog of Small Size National Bank Notes" is the only reference I have on hand, & he doesn't list this bank at all. There's a K&M publication, I think, a big telephone-book style, which attempts a comprehensive list of known notes by bank, denomination, & serial number. You might try that.

4 small known on the bank. you have the 5th and the 1st twenty.

Wow, that's great! Thanks for the answers. I've always wondered about this note and could never find anything on it. How common is it for banks to only have a few known notes?

Having just a handful known isn't uncommon. Many banks closed in 1930 or 1931 due to the depression, leaving just a small window for small size issues.

National Bank Notes, Sixth Edition lists it at $1500.

It has a yucky center crease that makes it lightly unattractive, but it is still a rare issue (according to the guide I own that lists the retail values for national currency). The amount of notes in existence to this date is in the single digits, meaning the note is very uncommon and probably holds a high three digit value.
