1976 D LMC Many Errors

Hello, just found another curiosity coin. It is a 1976 D LMC with multiple errors.
1. It may be an unlisted RPM. Couldn't find one similar.
2. Missing the F and most of the G in designer initials.
3. Missing the VDB completely.
4. Possible MD on OBV.

Any opinions would be great.

I agree with the Machine Doubling also making the mint mark machine doubled as well.

The FG I feel is a grease filled error. The G still looks pretty strong.

The VDB missing is common for some years. (1972 I believe)

The tops of the letters show die wear. ID "WE" The area next to the rim.
