Master die doubling would be my guess. Very common, on about 60% of those coins.

The "B" and the "R" look like Master Die Doubling but I cannot see it on IGWT. Maybe because the pictures are to pixilated or its a pretty worn out die. I'm fairly certain it is Master Die Doubling.
