Hi everyone. I found this in change and was wondering if it's an error. Notice above the 'A' in Canada the three indentations. Plus it seems like it might be off-center. I'm including a pic of the obverse just in case it helps. Thoughts?

It's not off-center enough to be called off-center, it's within Mint tolerance. It's the first time I see those marks though. My first instinct would be PMD.

The mark on the reverse does not appear to be related to the die.
