This looks like it could be a struck through or Lamination error, but I am leaning towards PMD of some sort. Any help out there? Thanks.

It's a lamination. I found one like that once.

It is labeled as a lamination peel, but the correct term is lamination.

It is a lamination peel. If it was damage it would have trashed "ERTY".
