Is this 1993p quarter lettering extra thick due to extreme die wear.

You are correct. Notice that the Die Deterioration is progressively worse towards the rim. The radial expansion of the planchet as it is struck is greatest towards the rim, this expansion is the cause of die wear.

There also heavy die abrasion to remove clashes as well.
