Is this something that happens often or is this usually a sign of someone tampering with the slab

It happens from time to time. The slab would be in pieces for someone to swap the label out

I've seen this with mixing up PR & MS and forgetting the mint mark.

Recently someone posted here a cc 20 cent piece labeled S mint sold for $300 instead of $600. I also posted pictures of an entire proof set labeled MS on all coins. A $1.00 gold piece sold at heritage auction labeled silver $ that's my favorite. When trolling for top 100 vams always look at slabs not only do they miss then many weren't valuable until recently.

seems pretty common from NGC.

It's not NGC. We're putting the wrong denominations on our coins, and they're just bringing our attention to that.

Does anyone have an NGC coin with a correct label?

i bought an 1832 bust half dollar in 2012 that was certified as PR genuine by PCGS. the coin was not a proof coin, it was a mechanical error by PCGS.
