If you are referring to a scratch that might be in the coin or case, take a strong flashlight and shine it at the coin, then move the flashlight around. If the scratch moves then the scratch is in the case, if it doesn't move then it is likely in the coin. Another thing to look for is if you can make the scratch cast a shadow elsewhere on the coin...again this would indicate it is in the case. You can do this without taking the coin out of the case. If you are referring to spots or milky/white blotches. Those are likely in the coin.
