Looks like it was a die gouge. The fields are on the outside part of the die. Moved incorrectly it can be damaged and show on the fields of the coins disappearing behind the other devices.

The hubs devices looks like a coin, the die is the negative where the devices are deep into the die and the fields out on the outside part of the die.
