Ok, it's not exactly an RPM -- well not even close but it is what is called Dropped Design (among other terms) due to the die cavity of the D being filled with debris (probably grease) and falling out like jello from a bowl onto a planchet and then being struck in.

what I love so much about Mr. Potters image is the though that there is the possibility that there are coins from this die that people think are philly mints.

Problem is in 1974 if you have a filled mintmark making it into a "P" coin no one would know it. What I find more interesting are the 1968, 69, and 70 "P" nickels from filled or polished away mintmarks that no one pays any attention to. (Philadelphia didn't make nickels in those years, but business strike coins with no mintmark do exist.)
