Here is another cool Mule Error! The obverse die should have been that of Queen Elizabeth II and dated 2005.

However, this one is a 2005 reverse die (Eisenhower) mated with a 2004 reverse die (KM#454 D-Day).

Two others minted like this are the General Patton and Roosevelt.

I'm not sure but 50% chance it was Sunshine Minting. I ordered 250 of these minted in 2005 from a company I represent (who holds the Cook Islands contract for these coins) for a large marketing firm that I supply. They sold them for several years before management decided to cut coins from their catalog. I bought back thousands of coins and medals they purchased from me over the years and a bunch of these were in the lot. (That really wasn't many coins as they had purchased well over a 500,000 coins from me over the years.)

Anyways this is the way they were struck and I guess they just sold them as "coins" and said nothing.

I doubt I can find out who the actual minter is as this is proprietary information. I can only guess.

I shoot General Patton and Roosevelt when I get time.
