Definitely Machine Doubling and the anomaly in the field by Washington's throat does not fit as a clash mark.

This is a pretty extreme case of Die Deterioration Doubling due to premature die fatigue due to improper heat treat or more likely the dies simply wearing out due to use. DDD is inherent to the extended use of dies and is not an error.
