I've seen other medals like this but they were brass and holed to wear, My question is, Was this plated when struck or was the silver plating applied at a later time? Would it get sent back from NGC as an altered piece?

Well, on the high spots I can see brass colour and the lower points don't look silver to me. I'd say it's not plated, but maybe it's because of the light and camera angle.

I also have an unholed one, but no sign of any plating on mine. It's a presidential campaign token from 1852. The DeWitt number is WS 1852-12
