Looks like there was some grease blockage on the "S". (note the device looks wider because the deeper part of the die was blocked on that letter)

The split you see if probably just Die Deterioration. (the direction is towards the rim)
Sometimes a slight coin contact can also affect the alteration on a coin. (when the rest of the devices look normal) I see no evidence of hub doubling on the other devices.
