I've had it in distilled water and scrubbed it with toothbrush bristles, my question is, does it need any more cleaning? and is that a break in patina on the cheek? Thanks

At this point I wouldn't do anything else to clean it.

Appears to have some lamination defects. I think more cleaning might do more harm than good.

I agree that it is good, cleaning-wise. Nice coin, though.

Definitely finished with the cleaning here. I love the Urbs Roma Commemoratives, awesome type.

Is it safe to put it in a 2x2 or a flip?

It is absolutely safe to do so. But I wouldn't recommend it and I'll tell you why. A couple others and I here on this forum believe that Ancient Coins should not be locked away in individual containers and slabs as such. Condition is not such a vitally important factor because they are so old. These are living pieces of history, and I would recommend that you keep them in a loose drawer or cabinet, and handle them with frequency, because Ancients are something to be handled and observed, not simply locked away.

Understandable. I kinda don't have a place for it raw. If I put it in a 2x2, then I look at all of them in my binder almost every day.

If you'd like it to continue getting cleaner, store it in a small closed container of olive oil. If the coin is solid enough to survive from this point, it shouldnt harm it to leave it in the soup. You might lose some of the green encrustation (though a lot of patina is missing, so perhaps its a good idea to leave the green in place).

I would not put it in Olive Oil. Olive oil is actually slightly acidic (about 1%) leaving it be won't do any harm

Nice coin, and good job so far.
It is always a judgement call when to stop, like a craftsman or an artist overworking a piece when trying for perfection sometimes you just have to call it a day. Not having so much experience with this I wouldn't know when that point comes but overcleaning is as bad as undercleaning (possibly worse because it can't be undone)
