There appears to be a deep indentation in that area. If it is indented, than I am thinking the "horn" is metal pushed up by the damage. Examination with a more powerful loop could answer the question. I recently purchased a 60x loop which has really been a big help in answering this kind of question!
Coop, I check it seems the normal design the forehead is attached with the horn In this coin it seems the forehead is missing material may be broken die?
I would also assume contact mark from something. Probably the same something that dented the rim. It's worth a penny so up to you if you want to keep it I kept a couple weird contact marks before and other examples of heavy die wear and die cracks. But the more I keep the more I go through and get rid of the worthless ones I used to keep hahaha
