Plenty of Doubled, Tripled, and Quad dies out there but I haven't seen a listing for a quintupled die. Here is a pic of one of my coins that I thought was a quad but now looks to be a quintupled die. The bottom serif of the R seems to show the five "punches".

I have a few but only on the modern coins like the Jefferson nickel 1956P WDDO-001 which is a quintupled die not sure of any others besides those 3 I mention?

but mind you is not that cool as the pup is 5 punch on bottom of the I in IGWT

There is also a Quintupled Die Obverse on a 1967 SMS Kennedy half dollar, 1950 Franklin and a 1964-D Kennedy half dollar
