This question came about during my last coin club meeting... Nobody could answer the mechanical process...

If anyone has a link to an article or some way of explaining the process I would be very helpful.

When the start or end of the sheet roll, the planchet is punched at the corner edge

Planchets are not cut a single row at a time. There are often two rows of planchets being cut simultaneously, with the second row offset from the first row, to minimize the waste with the scissel. If you look at a sheet of scissel, and imagine a slight slippage in the strip of metal being fed, double clips are easy to envisage.

Nope... but the scissel cutting process is probably different from US coins, whereby you see most corner clips.
