Im thinking its a 198_D Dime. It is very thin and weighs significantly less than 2.27 grams (can't remember what the exact weight is). Would this be a die adjustment or another type of error?

Looks like PMD to me. Road rash or someone took a grinder to it.

But would that cause the coin to flatten? The weight is 2.03 grams and is almost as thin as a piece of paper.

Definitely sanded down. The material that was sanded off accounts for it being under weight
