Well I'm no pro, but it just looks to me like some kid got bored and was playing with a power tool of some sort.

It is damaged. I have a few around here with the same type of damage. The obverse indicates that it was probably caught in a mechanism of a coin counting machine. I have also seen coins that were jammed in old style vending machines (Like a Gumball Machine) that when removed, look like this.

Maybe someone put a bottle cap on top of it and twisted it on top of the coin
