That is from a partial fill on the die with grease in those devices. It prevented a normal strike. The operator may have wiped the die and though it got most of the grease out of the die. He didn't. Thus the partial letters.

Note the image with the 'ICA'. You can see where the grease prevented the taper of the devices and left them looking wider. Just what you would expect to see on a Struck Through Grease error.
