I am wondering if anyone has any idea what might have caused this error? It weighs right (5g) but is wider (22mm) and thinner (1mm) than a normal Jefferson nickel. It also has GOD struck on the front. Any information would be greatly appreciated!

This is strange and will be somewhat speculative. It looks like two blanks had to be involved. The first strike was just the edge where you see "GOD" on the obverse which had to be resting on another blank planchet in the collar. There are no matching marks on the reverse just some weakness in STATES. The second strike would be the full strike with this coin in the collar. This is my best guess at what may have caused this. Cool coin indeed. Thanks for the post.

I had to look closer at this one:
An overlay of where the reverse should of had a second strike that was not there, flatten the coin a bit:

The post strike damage on the head was the first clue. The second clue was the flattening of the devices. Looks like a soft die squeeze job to make it was double struck. PSD

Soft die and if the diameter figure is right possible a little bit of pounding between pieces of leather to start "Texas sizing" it. Only two ways I can think of to increase the coins diameter and that would be Texas sizing, or a broadstrike, and it isn't a broadstrike.

I agree that the coin was altered. Proportional expansion of both faces is virtually impossible in a genuine error. And the extra GOD does look like it was added with a soft die.
Error coin writer and researcher.
