This does not look like a rim burr to me, since it is incuse rather than looking like a flange of "extra" metal.

I have seen this exact kind of rim damage on 25c straight out of plastic wrapped mint rolls.

I'm not sure what caused the damage, but it may have occurred after the coin was struck but before the coin left the mint. Once the coin is taken out of the roll it's impossible to prove it's not simply PMD.

It got whacked or pinched in one of the feeders after it was struck .. PMD.

I'm not too sure it's PMD, there appears to be a Blakesley effect opposite the gouge.
"Dipping" is not considered cleaning...

I thought the Blakesly effect is from the dies striking a previously clipped plantchet?

The Blakesley effect is often observed on clipped coins, however it can occur on any coin with an absence of metal on the horizontal plane of a blank as it is sent through the upsetting machine.

The burr was retained as well. You can see the outline through "NTS" in "CENTS".
