I know it's a scan, and you can't judge the surface, but can anyone tell me why this was marked at $20 at my LCS? Has it been harshly cleaned? Surely it wouldn't be marked at $20 if it was fake (do fakes come with Cuds and die breaks?).

Die cracks are almost standard for that issue. The Mint was having difficulties dealing with the hardness of nickle. As for the Cud that's a bit more interesting. Finally, as for being cleaned I can't tell. Scans don't lend themselves to opinions about that unless the changes are VERY obvious.

Hard to prove it was cleaned as scanned as older scanners don't show the true surface as compared to in hand, BUT I would go with cleaned. And " Cud" seems to be a hit in circulation, not a die chip! If you notice the rim is included in the damage, a " Cud" is a broken piece of the die on the rim area and would not show the actual rim !

I really can't tell much from those scans.
It appears that $20 would be very cheap given the apparent grade.

Die clashes, Cuds, and cracks were very common on these early nickel coins. Look at some 1865 3 cent nickels and you'll know.

I still think thats a Cud the die crack runs right up to it. If it was damage that would be an extremely convient place

But then I noticed that it seems to be integrated with the die crack that runs above the "UN". That puts me on the fence about it being a Cud.

It appears to be EF/AU and genuine. And yes, we cannot tell anything about the surface from the scans. You have it in hand. Is it cleaned?

I assumed it was cleaned when I bought it, but I am no expert on cleaning. The color seems normal.
