Is this an over-polishing issue or a filled die issue? Looks like over-polishing but the devices next to the missing/weak ones are not affected. It is a 1994, as just visible when the coin is angled just right.

Now that's just pitiful! You can see the die gouges from the polishing over the date, worse I've ever seen, usually die is retired by this point. The B looks like a filled issue, probably too mush polishing compound left on die in the recess.

Right below date by 45 degree gouge do I see a D?

I looked at this one a bunch of ways and could not see a D. Pretty sure it is plain.

The lines you are seeing where the date should be is rolling lines from the stock material. The missing devices are from a struck through error. (grease) Because of the grease in that area, these were not disturbed during the strike because of the full devices blocked.
