Mis-aligned die (MAD) and apparently some grease-fill areas on reverse.

MAD struck at angle causing a weakness in strike opposite side. Strength of strike was at 10 o'clock on obverse, on reverse that's 4 o'clock. Notice how weak strike is from 1-5 o'clock on reverse outer devices. The reverse is also VLDS and most likely grease filled as well.
