It's an AVE MARIA GRATIA type, not unlike footnote number 19 on this page. The coat of arms appears to be that of the Duchy of Burgundy. Like #19 there, it's probably of Dutch origin.

Sap, the reverse on the footnote no. 19 you refer to is too short, it looks quite like this one http://users.bigpond.com/gcates/jetons/jeton10.jpg

This is not Dutch. I see 2 fleur de Lys in the scutcheon and something that looks like the hat of a bisshop nat the flipside. I believe it is from one of the French Southern Papal cities as Perpignan, Avignon or anywhere in that region.

Burgundy perhaps?

I am still not sure, however this actually sold on ebay for 30-40 pounds, so it must have been rare!
