1979 Washington Quarter With No Face Structure, Letters Gone And Mint Mark

Here is the pic can you tell me if this was caused at minting since the edges are ok and the reverse seems to be ok too. I have been trying to post pics and it will not do it. Am I not able to do this from my iPhone 6plus I give up I'm frustrated with it. I will try something else if you can assist in uploading. Report this Post to the Staff
