It looks to be doubling caused by Die Deterioration. The letters and numbers appear mushy and flat.

MD happens on both sides and in multiple directions at times. May bounce to the left after moving up to the right, or a double bounce making two shelves. Have seen up to three shelves on a device! The mushiness of the obverse shows extreme die attrition, deterioration, wear, stretching polishing, overuse. Pretty strong die to not be exhibiting cracks!

Crazyb0, that is one heck of a Nickel that the OP has. Definitely has been overused, but the U.S Mint probably kept it going until the die wear got to be too much or until it broke! :)
