Would a broken puncheon create a raised area on the coin? It doesn't seem to me that it would.

Yes. The Metal is distorted on the punch. The part that should be taller on the puch is not pushed in the die. Leaving a void on the die, and now a raised area on the mint mark. (where it should have been a void) Many years have this issue, until.... the punch is replaced.
