Is this penny on the wrong planchet or is it off center or what is really the term for this mistake? On the obverse right side the rim is really thin and kind of sharp!

How does the edge look? I would bet the cent was put to a grinder

Look like some kind of mounting as an Encased Cent, but high pressure was used.

I meant to say the left side was sharp. As you can see there is copper that is raised in 2 spots on the edge rim part

Looks like a legit error to me. Off center strike? Tilted collar strike? Very nice pics by the way.

The penny bounces back and fourth between 2.6g and 2.7g and as you can see in the picture that it is 1/16th of an inch smaller in diameter.

There are many world coins that have been minted at US mints. Here is a link to a check list of them. Find which one matches yours.

When you look through that list check for coins minted in 1968 and 1969.A blank could have got stuck somewhere from 1968.

I can't remember when, But I did find a 64 rupe ( I believe) that was overstruck on a 1969 US cent. That might be an avenue to investigate!

Chances of a blank planchet floating around the mint for 5 years and then being struck is slim IMHO.
