I think that this is my first REAL find. I am sure of this one. It is so obvious. Would this be a one time mistake or repeated a few times?

to the CCF. To answer your question, it is not a one time event. I have a 1979 Susan B. Anthony dollar coin with the same thing. Keep searching and good luck!

Depends. If it was caused because of a MAD setup, then it might have happened several times. The weakness on the reverse makes me think it is also a MAD coin.

Are the areas where the black incuse marks on the edge where the collar clash is also on the rim?

I know you all are busy and I thank you for the time you have given my questions. I am learning SO MUCH very quickly. Sometimes it makes my head spin.

I was wondering is this a MAD collar clash? Is it something that I could sell for a few dollars or just really neat? It is in pretty good shape. How do you know if a coin is TPG grading worthy?
