@Docsrose, first welcome to CCF. Second, while the prospect of a two-mouthed Lincoln is a bit scary, it looks to me like his cheek has taken a well-placed hit, causing what sorta looks like a second pair of lips.

It's Post Strike Damage (happened after it left the U.S. Mint) and not an error at all. You can see the well placed hit moved some metal over and flattened it. Worth face value.
