It's a Temperance medal, from before the founding of AA. The Temeperance movement was the driving force behind the Prohibition.

I believe it refers to an article about this medal printed in the journal of the Token and Medal Society in 2004.

Another one of these medals is the first item on this page. According to that site, AWR are the initials of the issuer, Ashbel Wells Ridley.
