 To CCF! That "extra bottle" looks like a well placed hit to me. If so, then it's Post Strike Damage (happened after it left the U.S. Mint). On the hat, it also looks like it took a well placed hit as well.

The "bottle" doesn't look like a hit as it appears to be raised based on the lighting. Maybe a die gouge?  On the boy's hat is PMD, possibly a bag mark from the reeded edge of another quarter.

Spruett, the only thing with the "bottle" is there's too much glare to see what it actually is.

I agree that a clearer and closer image of the "bottle" is needed, but it displays the same directional shadows and highlights as the piers, suggesting it is raised. If it were incuse like a hit, the shadow/highlight relationship should normally be reversed. However, I've been tricked by this before in pics, so better pics are needed.

Yeah, a good and clear closeup would help. It would tell us what it is. As of right now, it's too tricky to tell what it is.

 Just checked my own quarter, the bottle is not there on mine and the pictures above look like a definite die problem. The hat is a hit.

Even if it is a small die gouge or something similar, I don't really see any premium value for it because the anomaly is so small. That's my opinion.

Here is the normal view of that aera:  Looks like the metal was chipped on and moved to that location from what I can see on the images. PSD. Damage to the coin.
