I do not know what type of damaged..PMD maybe? Both side has it?

Looks like Dremmel damage. If there would have been ghost images of the devices, I might have thought it was a lamination. But PSD it is.

Not a die clash. My first thought after seeing it was a possible struck thru. But like coop said, it looks like some type of drill damage.

I thought it was a Lamination at first, but after looking at both pictures, it some type of damage that happened after it left the U.S. Mint.
