I don't understand how a formation like this occurs. Has it been tampered with or is this somehow minting? I want to say it is 1942, but is there anyway to know for sure? Any advice anyone could give on ways to know in the future would be awesome.

It looks like a 42 to me, unfortunately it's just been damaged, it's not an error.

To the CCF.

Certainly didn't leave the mint that way.
Coin has been badly damaged somehow after it left the mint,
known as PMD (Post Mint Damage)
Rest in Peace

Yes it's a mangled 1942-S . If it was a 43 it would be a steel cent.
BTW , To CCF , .

Thank you so much. Yeah it just sparked my interest when I was sifting through some old change. I know nothing about errors since I don't deal with them much in my collection. Always good for a second opinion instead of just throwing it aside.

It's a very damaged 1942 S LWC. It's worth Copper melt value now.

Something hit the edge of the coin, part way up, and shoved metal sideways into the date.
